Home Health Licensed Practical Nurse (LPN) : Full Time & Flexible

Pathwell Health - The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records.
May advise patients on health maintenance and disease prevention or provide case management.
Licensing or registration required.
ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential functions include, but are not limited to the following:
Provide skilled nursing care under the direct supervision of the Clinical SupervisorComplete evaluation tasks, including vital signs and medication reviewsCoordinate with physicians, therapists and other individuals in the client's care planAssess and chart observations of the client's condition at each visitMaintain complete and accurate documentation of nursing careWork collaboratively with all members of the health care teamProvide education to clients and families on proper home health care procedures and strategiesRecord patients' medical information and vital signs.
Monitor, record, and report symptoms or changes in patients' conditions.
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plansModify patient treatment plans as indicated by patients' responses and conditionsMonitor all aspects of patient care, including diet and physical activityAssess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problemsPerform other duties as assignedMINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)Associates degree required; Graduate of an accredited state approved, Bachelor's Degree from school of nursing preferredCurrent and valid Virginia Licensed Practical Nurse licenseOne year of LPN experienceCurrent CPR certification (required) , Driver's License , Car Insurance , Car RegistrationStrong interpersonal skills that build personal rapport and establish trust with the clientComputer literateStrong organizational skillsExcellent written and communication skillsAbility to meet physical demands of the job (ex:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
